At Umpqua Valley disAbilities Network (UVdN), we support individuals with disabilities as they navigate important life transitions. From moving out of parental or group housing to transitioning from long-term care facilities, we provide personalized assistance for greater independence. Our approach includes life skills coaching, community resources, and developing individualized plans that promote informed choices and self-reliance.
